Michiana Eye Center files a wide range of insurance plans.

We recommend providing both your medical and vision insurance information in order for us to best serve your eye care needs.

Insurance companies have divided eye examinations into two categories: Those that are conducted to check for refractive error, and those that are medically necessary.

Vision exams

Vision exams are considered routine wellness exams for the eye. Vision insurance often covers these exams, including the test that determines refractive error (nearsightedness/farsightedness and astigmatism) on an annual or bi-annual basis.

Vision plans often contribute toward:

  • Vision Exam: partial or 100% paid for (plan dependent)
  • Eyeglasses: a pair every year (or every 2 years)
  • Contacts: monetary benefit (usually covers 6 months)

Common Vision Insurance plans include:

  • Vision Service Plan (VSP)
  • Eyemed
  • Unitedhealthcare Vision
  • Spectera
  • Vision Care Plan
  • Superior Vision

Medical exams

Medical exams are medically necessary exams conducted to diagnose and treat diseases of the eye. The doctors at Michiana Eye Center will use both their clinical experience our state-of-the-art diagnostic tests to evaluate, monitor and treat ocular diseases including glaucoma, diabetic retinopathy, macular degeneration and many other medical conditions. Since the eyes are the windows to the rest of the body, many systemic diseases can also be detected through a medical exam.

Medical insurance may contribute toward your eye exam and diagnostic tests, but often do not cover refractions (the part of the exam when the doctor asks “which is better, one or two”). Co-pays, co-insurance, and deductibles will still be collected when services are rendered.

Even if your exam is billed to your medical insurance, you may still be able to use you vision benefits to contribute toward your purchase of glasses or contacts.

Common Medical Insurance plans include:

  • Medicare
  •  Indiana Medicaid
  • Anthem Blue Cross Blue Shield (BCBS)
  • United Health Care (UHC)
  • Humana
  • Select Health
  • Community Health Alliance
  • Sagamore
  • Aetna
  • MDwise

Please contact the number listed on the back of your insurance card if you have any questions on your specific policy, or to find out if Michiana Eye Center is a provider.
